TES in Cambodia

Monitoring antimalarial drug resistance is a priority activity of the National Centre for Parasitology, Entomology, and Malaria Control (CNM) of Cambodia to ensure that the malaria treatment policy being implemented is evidence-based and that early deterioration in the efficacy of recommended treatment regimens is identified in a timely fashion and those drug policy recommendations updated. TES with various antimalarials has been carried out in selected strategically located sentinel sites in Cambodia since 2008, using the WHO standardized 28/42-day in vivo protocol to generate comparable information from different sites across the country. Molecular assays and genotyping for drug resistance markers such as Kelch 13 (K13), Pfmdr copy number and recently, Plasmepsin 2-3 complement the in vivo data. Molecular markers surveillance has a role in ascertaining the extent of resistance. Still, it must be interpreted in the context of clinical and parasitological data to drive treatment policy decisions, hence the importance of the in vivo TES.

The Challenge to TES and shift to iDES

The success of intensified elimination interventions reduced the burden of both P.f and P.v malaria. However, the declining number of malaria cases also poses technical challenges to TES. An insufficient number of cases that meet enrollment criteria make analysis and interpretation of TES result difficult, resulting in a long time to complete the studies and a change of study sites that entails more financial and human resources and retraining when new sites are selected.

Where very low caseload in elimination areas, WHO recommends countries with robust surveillance systems for malaria to integrate the therapeutic efficacy with the robust surveillance system, i.e., integrated Drug Efficacy Surveillance (iDES).

Position Summary

Purpose/Specific Objective of the Activity

To ensure the expected outcome of the implementation of the iDES as planned.

Objectives:

  • To support the country in the rollout of iDES training
  • To support the government in the implementation of iDES in the pilot provinces in 2022 and expanding to the rest of the country in 2023
  • To support the monitoring and supervision of iDES
  • To support the data collection, analysis and reporting

Description of activities to be carried out

  • To support CNM and partners in planning and adjustment of iDES activities based on the number of cases reported by health centres
  • To support the training and cascading training of iDES from national to health centres and VMW/MMW in the pilot areas of 9 provinces in 2022
  • To support the CNM and partner implementors (CMEP, CHAI and CRS) in monitoring and supervision of the implementation of the iDES in hospitals, health centres and VMW/MMWs
  • To support the establishment of capacity in shipment, storage and processing (reading slides and testing of DBS) of samples
  • To support the establishment of the capacity of designated hospitals and operation for iDES
  • To support the collection and reporting of the iDES data and information through the iDES app in MIS
  • To support the monthly iDES meeting
  • To support organizing a six-monthly progress review and planning meeting
  • To communicate with implementation and funding partners about the progress of the iDES
  • To help the CNM and implementation partners to draft six-monthly iDES reports and write a thesis for publication
  • Any other related matters

Expectations - Duties, Responsibilities
  • Output/s
  • Output 1: Completion of national and provincial cascading training of iDES: to support the country in design and implementation of the iDES training at national level and in the 12 initiation provinces and expanding to the rest of the provinces
  • Deliverable 1.1: support provided in organizing and management to CNM and partners (CHAI and CRS) in completion of the national launching and TOT for iDES in later September or early October, evidenced by a report of the national launching and TOT workshop
  • Deliverable 1.2: support provided in organizing and management to CNM and partners (CHAI, CRS and CMEP/C) in the cascading training to VMW/MMW in the second and third week of October in the 12 initiation provinces, evidenced by 12 reports of the provincial cascading training to VMW/MMW
  • Deliverable 1.3: support provided in organizing and management to CNM and partners (CHAI, CRS and CMEP/C) in the cascading training to VMW/MMW in the first quarter of 2023 in rest of the provinces, evidenced by at least 8 reports of the provincial cascading training to VMW/MMW
  • Output 2: Routine and reactive implementation of iDES as planned: support the implementation of iDES as a routine part of the national malaria surveillance and response system
  • Deliverable 2.1: Monitoring the implementation of health facilities and VMW/MMWs in iDES through the established MIS iDES dashboard, together with country counterparts, and alert CNM, WHO, CHAI, CRS and CMEP if there are 1) discrepancy of number of cases enrolled and number cases reported by VMW/MMW and health facilities, 2) non-logical data reported, 3) other issues calling attention
  • Deliverable 2.2: Routinely on-site supervise the implementation of the iDES as per schedule together with counterparts or partners or independently in the health facilities and/or VMW/MMW planned in implementation of iDES
  • Deliverable 2.3: Reactively on-site supervise the implementation of the iDES in the health facilities or VMW/MMW with identified or potential problems based on the monitoring of MIS iDES dashboard and information from other sources such as Pv Radical Cure
  • Deliverable 2.4: Routinely and reactive monitoring the implementation of blood slide reading, storage and shipment of DBS at designated hospital laboratories through iDES dashboard and on-site visits; collaborate with CNM laboratories for support the
  • Deliverable 2.5: facilitate a swift shipment of blood slides and DBS to designated laboratories for PCR testing when potential treatment failures are detected
  • Output 3: prompt analysis of iDES data for decision making and results sharing with the national malaria program, malaria partners in countries, at regional (GMS) and global level (WHO)
  • Deliverable 3.1: support the counterpart of the country in organizing monthly iDES meeting for iDES implementors to report the results using standardized report template and presentation template
  • Deliverable 3.2: support the counterpart of the country in organizing six-monthly iDES meeting to discuss the achievement, challenges facing and solutions
  • Deliverable 3.3: support the counterpart of the country in consolidating iDES data and information and write annual report, and turn the annual report into publishable papers for dissemination of the outputs of Cambodian iDES
Requirements - Skills, Qualifications, Experience
  • Qualifications required:
  • Education:
  • A University degree or higher in medicine, public health, pharmaceuticals or related areas
  • A Master degree in science in epidemiology or public health is an advantage
  • Experience required:
  • Experience at least three years in malaria elimination and/or control management
  • Experience in data collection, reporting, analysis and presentation
  • Experience in working in or with national and provincial health systems in Cambodia
  • Experience in drug efficacy monitoring is an advantage
  • Skills / Technical skills and knowledge:
  • Excellent personal communication skills that fit the local context
  • Skilled in using MS office
  • Skilled in using statistical software such as Stata
  • Skilled in quality report writing
  • Language requirements:
  • Fluent in speaking and writing in English and Khmer
