Scope of Work

FHI 360/Cambodia is seeking reputable and registered suppliers to provide 266 Units of VMW signbord, 15 units of Billboard and service delivery and installation to support EpiC target provinces, including Battambang, Pailin, and Pursat. These materials have been identified as priority needs for 2026 to ensure the effective delivery of lifesaving services by village and mobile malaria workers (VMWs/MMWs) to malaria-vulnerable populations. The services include community health education, malaria screening and testing, case referral for treatment and follow-up, as well as proper recording and reporting. The provision of these materials will also help ensure adequate safeguarding measures for both service providers and beneficiaries.

FHI 360/Cambodia expects to award a fixed price Purchase Order with specific deliverables and payment schedules. Offeror shall follow the instructions contained herein and furnish all information as required. Failure to furnish all information requested may disqualify a quotation. A responsive offer is one that fully complies with all terms and conditions of the solicitation without modification.

Preparation of Offers

All quotation packages must be submitted in English language, single-spaced and numbered consecutively. All offers must be signed by an individual authorized to commit the offer on behalf of your firm/company. The completed forms shall have no erasures except those necessary to correct errors made by the offeror, in which case such corrections shall be initiated by the person or person(s) signing the offer.

The Offer (Required documents)

Only quotations submitted in accordance with this RFQ requirement will be eligible for evaluation. The quotation package must include the following documentation:

  • Cover Letter
  • Company profile
  • Company Certificate: Business registration, patent certificate, and VAT certificate

The company profile should include:

  • Name, date of establishment, contact details, and other relevant details;
  • Affiliations with other companies/organizations
  • Statement of capacity to deliver requested service on the date and time/period indicated
  • A copy of Registration of business certificate, patent certificate, and VAT certificate.
