Purpose
To support the objectives of the project, FHF invites legally registered private businesses to submit proposals for the supply of ophthalmic equipment to designated facilities across 12 target provinces and 3 target locations in Phnom Penh. The quantities and technical specifications of the required equipment are detailed in the list below.

Legal and Registration Requirements
Bidders must be legally registered companies or businesses. Copies of valid business registration certificates, patents, and other relevant legal documents must be included in the proposal.
Training Equipment Provision
Bidders must ensure that all required training equipment is delivered to each designated recipient no later than two (2) weeks after the delivery of the main equipment.
International Bidders
International bidders must clearly describe their logistics and customs clearance plan, including how they will manage port clearance and shipment of the goods to the specified locations.
Scope of Bid and Supporting Documentation
Bidders are encouraged to submit proposals that, where possible, cover the complete set of items listed in this RFP. Each submission should include clear images of sample products, and bidders are encouraged to provide website links or online product references to support their proposal.
Alternative Options and Recommendations
If bidders wish to propose alternative equipment options, they should provide detailed recommendations specifying the model, price, quality, technical specifications, country and company of origin, warranty period, and any other relevant information for each proposed item.
Product Availability and Delivery Schedule
Bidders must indicate the availability of the proposed products and provide an estimated delivery schedule, including expected arrival dates for items that may not be available for simultaneous delivery.
